Personal Training and Consultations
- Initial counseling to discuss your goals, interests, needs, and medical history - Lifestyle counseling (review of what you are doing now for physical activity and what is the most effective program design for your personal training sessions) - Program design (each personal training session will be a combination of one or more of the following components: cardiovascular training, muscle strength and endurance training, flexibility training) - Modes of training (your personal trainer will choose the most effective mode of training and use of equipment based on your needs and goals) - Question and answer period (throughout your session, you will have the opportunity to ask your personal trainer about facts, fallacies, or controversial issues pertaining to exercise and program design)
- That there are potential risks (i.e., episodes of transient light-headedness, muscle soreness, abnormal blood pressure, chest discomfort, leg cramps, and nausea, and extremely rarely, heart attacks) and that I assume willfully those risks; - My understanding that I may ask any questions or request further explanation or information about the procedures at any time; - That I have read, understood, and completed the Pre-Training Questionaire and that the answers to all the questions were negative or if I answered positive (yes) to any of the Pre-Training questions, I have attached a written medical consent with this form; - That if I need to cancel or reschedule a scheduled appointment, I will provide at least 12 hours notice on 0422 914 632 to my personal trainer. If I do not show up for a scheduled appointment or cancel within 12 hours, I will be charged my session rate for the missed appointment.
In agreeing to participate in personal fitness training and/or consultation, I accept all responsibility and waive any legal recourse against The Naked Truth Fitness Solutions and its staff from any and all claims resulting from personal injuries sustained. This waiver shall be binding upon my heirs and my personal representatives.
I agree to be changed and ready to start my exercise session at the scheduled appointment time. Appointments that begin late will end based on the scheduled start time. All personal training sessions are 45 minutes in length.
